According to the Law on the Protection of Personal Data No. 6698 (“Law No. 6698”), your personal data may be processed by Çelebi Dental Oral and Dental Health Clinic (“Institution”) as the data controller within the scope explained below.

Your collected personal data will be used for the activities carried out by the Institution;

3) To Whom and For What Purpose Can the Processed Personal Data Be Transferred

Your collected personal data; in line with the realization of the above-mentioned purposes; It may be transferred to our business partners, suppliers, legally authorized public institutions and private persons or organizations and third parties within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of Law No. 6698, and may be processed domestically or abroad.

4) Method and Legal Reason for Collecting Personal Data

Your personal data is collected by our Institution through call centers, website, mobile applications, our hospitals and polyclinics in a physical environment, completely or partially automatically or non-automatically, provided that it is part of the data recording system. Your personal data collected with these methods may be processed within the scope of the personal data processing conditions specified in Articles 5 and 6 of Law No. 6698.

5) Rights of the Personal Data Owner Listed in Article 11 of Law No. 6698

As personal data owners, if you submit your requests regarding your rights to our Institution using the methods set forth in the FR.KY.019 Data Owner Application Form, the Institution will finalize the request free of charge as soon as possible and within thirty days at the latest, depending on the nature of the request. However, if the transaction requires an additional cost, the fee in the tariff determined by the Personal Data Protection Board will be charged by the Institution.

*In this context, personal data owners have the right to:

Learn whether their personal data has been processed,

Request information about the processing of their personal data if it has been processed,

Learn the purpose of processing their personal data and whether it is used in accordance with its purpose,

Know the third parties to whom their personal data has been transferred domestically or abroad,

Request correction of their personal data if it has been processed incompletely or incorrectly and request notification of the transaction made in this context to the third parties to whom their personal data has been transferred,

Request deletion or destruction of their personal data if the reasons requiring processing are eliminated, despite the fact that it has been processed in accordance with the provisions of the Law and other relevant laws, and request notification of the transactions made in this context and in the event that their personal data has been processed incompletely or incorrectly to the third parties to whom their personal data has been transferred,

Object to the emergence of a result against the person by analyzing the processed data exclusively through automated systems,

Request compensation for the damages incurred due to the processing of their personal data in violation of the law.
